Fatigue is the most common complaint in primary care medicine.

More common than pain. More common than anxiety. More common, even, than the cold.

And the most common response from the medical system? A blood panel, a TSH test, and sometimes — when everything comes back "normal" — a shrug. Maybe a referral to a psychiatrist. Maybe the implication that it's stress, or that you just need to sleep more.

But you are sleeping. And you're still exhausted.

Here's what I've found in two decades of integrative practice: "normal" lab results don't mean optimal function. And fatigue virtually never has a single cause. It's almost always a convergence — several factors colliding to produce a body that cannot generate or sustain energy the way it was designed to.

Let me walk you through the real causes.

1. Mitochondrial Dysfunction

Your cells contain hundreds to thousands of mitochondria — the organelles responsible for converting food and oxygen into ATP, the currency of cellular energy. When mitochondria are damaged or depleted, the fundamental energy-production machinery of every cell in your body runs below capacity.

Mitochondria are exquisitely sensitive to oxidative stress, environmental toxins, nutritional deficiencies (especially CoQ10, B vitamins, magnesium, and iron), and certain medications (including statins, which deplete CoQ10 by up to 40%).

This is why some people are exhausted despite "normal" thyroid function and "normal" iron levels. The problem is further upstream — at the cellular energy-production level.

2. HPA Axis Dysregulation (What People Call "Adrenal Fatigue")

Your hypothalamic-pituitary-adrenal (HPA) axis is your stress response system. Under chronic stress, it runs in a state of sustained activation. Cortisol is chronically elevated, then eventually blunted as the system becomes dysregulated.

The symptoms of HPA axis dysregulation look exactly like general fatigue: tired in the morning despite adequate sleep, second wind at 10 PM, difficulty staying asleep, low motivation, brain fog, craving salt and sugar, sensitivity to stress that used to feel manageable.

"Adrenal fatigue" as a lay term is controversial — the adrenal glands themselves rarely fail in otherwise healthy people. But the HPA axis dysregulation it describes is real, well-documented, and extremely common.

3. Blood Sugar Instability

The energy crashes that most people experience mid-morning and mid-afternoon are blood sugar crashes. When blood glucose drops precipitously after a high-carbohydrate meal, your body spikes cortisol and adrenaline to release stored glucose — which feels like anxiety, shakiness, and profound fatigue.

This cycle repeats throughout the day for people eating a standard diet, producing a roller-coaster of energy that leaves them reaching for coffee and sugar just to function.

Stable blood sugar — achieved through protein-rich, fiber-dense, lower-glycemic meals — produces stable, sustained energy. This connection is among the most underappreciated in medicine.

4. Nutritional Deficiencies That Look Like Fatigue

  • Iron-deficiency anemia: The classic fatigue cause — but even low-normal ferritin (stored iron) without true anemia significantly impairs energy and cognition
  • Vitamin D deficiency: Affects energy directly through its role in mitochondrial function and muscle metabolism; over 40% of Americans are deficient
  • Vitamin B12: Essential for red blood cell production and nervous system function; especially common in vegetarians, vegans, and people over 50 with declining stomach acid
  • Magnesium: Required for over 300 enzymatic reactions, including ATP synthesis; widely deficient due to soil depletion and modern diet
  • CoQ10: Essential mitochondrial nutrient that declines with age and statin use

5. Poor Sleep Architecture (Not Just Duration)

You can sleep eight hours and still be profoundly fatigued if your sleep architecture is disrupted — if you're not reaching adequate deep sleep, or if sleep apnea (which affects an estimated 30 million Americans, most undiagnosed) is fragmenting your sleep cycles.

A sleep study or at-home sleep monitoring device is worth pursuing if fatigue persists despite adequate sleep duration.

6. Thyroid Dysfunction Beyond TSH

As discussed in our hormone article: conventional thyroid testing often misses the full picture. Optimal TSH is not the same as adequate T3. Hashimoto's thyroiditis — an autoimmune thyroid condition — can cause significant fatigue with intermittently normal labs.


Exhaustion is a signal, not a sentence. Your body is telling you that something in the system needs attention.
